Important Disclosure: The HealthKarma PersonalCare Benefit’s program powered by Benalign is NOT Insurance, not intended to replace insurance, and does not meet the minimum creditable coverage requirements under the Affordable Care Act. It provides direct access to providers that charge a zero co-pay for their services, or in the case of the acute and chronic Rx benefit, a small copay based on the generic medication elected. Certain services are provided by third-party vendors. Benalign is only offered in states where an active business license is in force. All services are designed to be offered to adults over the age of 18 as the primary Benalign member. Virtual care services and any resulting prescriptions are subject to the discretion of the licensed physician. Prescriptions will not be available for DEA-controlled substances, non-therapeutic drugs, or certain other drugs which may be harmful because of their potential for abuse. State Availability: The program is only offered in states where Benalign has an active business license in force, which currently are: Alabama, Arizona, Arkansas, California, Colorado, Florida, Georgia, Idaho, Illinois, Indiana, Kansas, Kentucky, Louisiana, Michigan, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, New Jersey, New Mexico, New York, North Carolina, Ohio, Oklahoma, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, Utah, Virginia, Wisconsin, and Wyoming.
